Ambientado en las saturadas calles de Kowlong, inundadas por la oscuridad y el caos, este juego de acción, aventura y combate pone a los jugadores en la piel de un hyoki, una entidad carente de memoria y forma física. Su único objetivo es erradicar a los monstruosos seres conocidos como cefalófagos, que deambulan por la ciudad haciéndose pasar por humanos.
Mientras recorren el increíble entorno urbano iluminado con luces de neón, los jugadores deberán buscar aliados entre los humanos conocidos como singularidades, infiltrarse en peligrosas organizaciones y entrar en combate aprovechando el poder de la sangre. A medida que se desarrollan el drama y el suspense, profundizarán en el misterio que entrañan tanto la existencia del hyoki como la aparición de los cefalófagos.
Una entidad conocida como hyoki se halla vagando en el caos de Kowlong, acosado por los enigmáticos monstruos conocidos como cefalófagos, que imitan a los humanos. Aunque carece de recuerdos, su intención es firme: erradicar hasta la última de esas criaturas. Por ello decide infiltrarse en la ciudad y emprender la misión de dar caza a los cefalófagos que acechan ocultos entre la gente.
Desprovisto de forma física, el hyoki es capaz de poseer y controlar cuerpos humanos a voluntad, convirtiendo a la gente que deambula por las calles en sus peones. Gracias a esta habilidad, puede atravesar barreras, asumir la identidad de personas concretas e infiltrarse en zonas y organizaciones peligrosas, además de cruzar la ciudad a gran velocidad cambiando rápidamente de huésped.
Aunque los cuerpos humanos son frágiles, el hyoki puede consumir su fuerza vital para convertir su sangre en armas y enfrentarse a los feroces cefalófagos. Hasta cuando está gravemente herido, es capaz de seguir luchando cambiando eficazmente de huésped. Al poseer a singularidades, determinados humanos que comparten una alta sincronía con él, el hyoki es capaz de desatar ataques aún más poderosos y de activar diversas habilidades. La relación de cooperación con estas singularidades es la clave para desvelar los misterios cada vez más oscuros de la ciudad de Kowlong.
